WHAT TO BRING

1) Please bring your photo ID, health insurance card, Referral Form / Doctor’s prescription, X-rays, MRI, or Surgical reports.
2) Wear comfortable loose fitting clothing.
3) We encourage you to fill out our Patient /Medical History Form to save time on your initial consultation. Please arrive 15 minutes early if filling out the forms at the office.
4) Make a list of questions that you want to discuss with your therapist on your first visit.
5) Note any signs or symptoms that could be of importance to your related injury.

INITIAL EVALUATION

Your Initial Evaluation may include the following assessments: range of motion, postural alignment, gait analysis, palpation, muscle strength/endurance, coordination and changes in neurological function. Your therapist will address your concerns and begin treatment to ensure the quickest recovery and best results. Open communication helps us understand your condition. Please indicate if anything is causing you pain during your evaluation. We record your progress throughout the rehabilitation process, modifying a plan of care and trying new treatments as needed. The number of visits will depend on how your body is responding. Recommendations between you and your physical therapist will be discussed and submitted to your referring physician for approval.

HEALTH INSURANCE INFORMATION

Lahaina Physical Therapy participates with many health insurance providers: In order to have your health insurance provider cover the costs of physical therapy (coverage amount depends on your individual health plan), you are required to have a Referral from your physician. Your referral can be faxed to LPT directly at 808.661.5264 or you can bring it to the office. Hawaii is a direct access state, therefore if you would like to pay privately, you do not need a referral.
